I hereby grant permission for my child(ren) to be photographed and/or videotaped during Faith Bible Church's Vacation Bible School program. I understand that these materials may be used for promotional purposes, including on the church website and social media - no child will be identified by name. I release Faith Bible Church and its volunteers from any liability arising from the use of these photographs or recordings
I understand that participation in Vacation Bible School activities involves inherent risks, including the possibility of injury, sickness, or other harm. I hereby: Release and waive Faith Bible Church, its directors, volunteers, and agents from any and all liability, claims, or demands for accidental personal injury, sickness, death, or property damage that may be incurred by my child or myself during VBS activities. I agree to assume all risks of accidental personal injury, sickness, death, or property damage as a result of my child's participation in VBS. I consent to any medical treatment deemed necessary by Faith Bible Church staff in case of injury or illness.
